I don't understand this distinction:

'There are two kinds of relevant “crazy.” One is psychiatric illness—which I believe exists because I have seen it up close and personal, and I don’t believe demonic possession is the only cause. The second is often also psychopathology, but more firmly rooted in problems of personality functioning—identity, self-direction, empathy, and intimacy. It is the kind of crazy people make true crime podcasts about.'

I've described my own struggles with mental illness, and I wonder which you think I am. https://jonahogilwy.substack.com/p/if-i-were-that-guy-i-would-scream?r=loua

I'll take a read,--But as a physician, I can't really apply on which "type of crazy" someone is who is not my patient! My point here is really about the difference between personality pathology and Psychiatric illness As it is understood in the world. These are both of course psychiatric illness at the end of the day, but narcissistic personality pathologies tend to cause dysfunction in others as well.

So the distinction you're making is between non-narcissistic and narcissistic disorders? The distinction is even less clear to me after your response.

I'm indebted to psychology/psychiatry for the diagnosis and treatment of schizotypal personality disorder. I'm not sure that's what I have, but the very notion that I had a personality disorder was crucial to me in making sense of my strange and painful past. This is explained in the essay, which I would be honored if you would read.

I might well be interested in doing that. I will think about what I might write, and if you have any ideas, please let me know.

The way we relate to personality disorders for ourselves and the way we relate to non-self individuals are very different. It's a great essay. There is a MASSIVE difference when we use the term "crazy" about OTHERS. So it's not so much personality disorder vs. personality disorder. It's who we are talking about--ourselves or our patients. Our attitudes about others? Vastly different.

I hope that is helpful?
